On-the-Job Illness or Injury?
Workers' Comp's New Web Pages, Downloadable Forms

For those who become ill or injured on the job, here's a place to start -- the updated Web pages of campus's Workers' Compensation and Vocational Rehabilitation Office.

Information available on the Web site includes a revised Workers' Compensation Handbook for Supervisors and an employee information section -- parts of which are available in Spanish in addition to English.

Employees may print the handbook and a number of downloadable forms, including the Employee's Claim for Workers' Compensation Benefits (DWC Form 1) and the Employer's Report of Occupational Injury or Illness (Form 5020).

To visit the Web site, go to www.uhs.berkeley.edu/FacStaff/ and select "Workers' Compensation." Those without Web access may call Fumi Nihei at 643-7921 to receive a copy of the handbook.
